Key Responsibilities
Identify non-clinical barriers to patient engagement in treatment
plans
Coordinate support services and community-based resources for
patients
Provide individual consultation and follow-up based on Social
Determinants of Health
Required Qualifications, Training, and Education
High school diploma or equivalent
Community Health Worker Certification by an approved training
program or initiated within 1 year of hire
1-3 years of experience working with underprivileged
populations
Specialized knowledge in addressing barriers to healthcare
